A well-established NDIS and aged-care provider in Sydney is seeking an Occupational Therapist to deliver home, school and clinic-based assessments across the inner west and northern suburbs. Enjoy a genuine hybrid model — two days from home for reporting and planning, three days out in the community — with a supportive senior team and a manageable, capped caseload.

Responsibilities

• Complete functional capacity, home modification and assistive technology assessments
• Develop and review NDIS therapy plans in collaboration with participants and support coordinators
• Deliver 1:1 intervention across paediatric, adult and older adult caseloads
• Prescribe and trial assistive equipment and minor home modifications
• Maintain timely, high-quality clinical documentation and NDIS reports

Requirements

• Current AHPRA registration as an Occupational Therapist
• 2+ years post-graduate experience in community, NDIS or aged care
• Current Australian driver's licence and reliable vehicle
• NDIS Worker Screening Check (or willingness to obtain)
• Excellent report writing and interpersonal skills
